Manual processing takes time from care
Transcripts, intake forms, referrals, discharge summaries, research documents, and policy SOPs still get read, retyped and filed by people. That work is slow and it comes out of time for patient care and other relevant work.
All generated notes have to be re-checked
AI scribes, summarizers and document processors write new text. That text can have a detail nobody said or wrote, or drop something important. The only way to know and be sure is to compare the entire note output with the source again.
How it works
AI is used only to understand content and its meaning. Facts are never AI generated, they are extracted verbatim from the source.

Each part is labelled according to its meaning, e.g. critical, key point, an action, a warning, data, etc. The same input processing can be re-ordered according to need, e.g. by priority, by type, or in a clinical template such as a SOAP note.

Every item is checked automatically by code: Intellean’s verification is not AI. Each item is matched against the source as an exact string, by code.
Sign-off is permanent and tied to the exact report it covers.
Review
Signed off and locked for the record
A reviewer only needs to correct what the verification check flags, reducing review and verification load. The changes are logged and the sign-off is stored with their account, the time, and a fingerprint of the exact report and source. These are immutable.
What it handles
Visit transcripts
The content from recorded or dictated encounters is extracted verbatim and made into the SOAP note, GP consultation, cardiology consult, discharge summary, surgical progress note or whatever is needed. Nothing is dropped or omitted, every passage and detail of the transcript preserved.
Forms and statements
Every detail from intake forms, EOBs, research charts, and other similar documents is extracted as is. They become key fields and line items, which are calculated by code, and marked and verified as calculated.
Clinical, policy and research documents
SOPs, guidelines and papers can be instantly arranged by section, priority or type. Tables, figures and equations are kept as images cut from the original page, so nothing is retyped or invented.

Permanent sign-offs. Sign-offs and corrections are added to a permanent log, which can’t be changed or deleted afterwards.
Changes after sign-off are detected. The report displays any changes made to it or its source after it was signed off.
